Ministry Activity Room Request Form

This form is for reserving space at the Damascus Community Church building for ministry-related activities and events.

All room requests must be made a minimum of 48 hours in advance of event.

Upon submission, your request will be forwarded to the Church Office and reviewed for availability.  You will be contacted to receive approval or denial of your request. If your request is denied, our staff will do our best to work with you to determine other options suitable for your event.

*If using the Kitchen, please note the following:
- We can provide the following select items if we have an estimate on the number of people attending your ministry event. (plates, cups, napkins, utensils, coffee, cream, and sugar.)
- Any other items are your responsibility to bring AND take with you when you leave.
- Please do not leave any leftovers in the Kitchen after your event. (Unfortunately, your kind intentions typically end up becoming a science experiment others have to throw out.)
-Please dry and put away your dishes.
-Please clean the kitchen at the end of your use. Try to leave it in better condition than you found it.
